                                     2. Marriage Certificate (if deceased victim is married)

                                     3. Medical Certificate (if victim survived the HRV)

                                     4. Death Certificate (if victim died due to HRV)

                                     For Financial Assistance for Rehabilitation

                                     1. Birth Certificate (if victim is minor)

                                     2. Certificate of Detention (in case of unjust imprisonment)

                                     3. Certificate of Release (in case of unjust imprisonment)

                                     4. Judgment of Acquittal (in case of unjust imprisonment)                                                

Schedule of Availability of Service             :           Monday to Friday, 8:00 AM to 5:00 PM

Total/ Maximum Duration of Process         :           Regular Financial Assistance

                                                                                    - 44 days and 42 minutes, if there is
                                                                                      finding of HRV by Regional Office

- 83 days and 42 minutes, if there is no finding of HRV by Regional Office but the Commission en banc determines otherwise
